Community COVID-19 Impact Survey

Take SDC’s Community COVID-19 Impact Survey

The Social Development Commission (SDC) is excited to launch our 2022 Community Needs Assessment.

The COVID-19 pandemic has impacted us all, from our personal lives to our professional lives. Although it has been more than two years since the start of this pandemic, we are still dealing with changes in how we live and work. We want to know how the COVID-19 pandemic has impacted your household or business.

Complete the COVID-19 Impact Survey by Wednesday, June 15th, to express how COVID-19 has impacted your household or business.

Your Participation is Very Important to SDC!

Your voice is important! Here is your opportunity to share your experience and inform decision-makers. Help us understand in what ways we can improve access to resources and other needed pandemic responses.

Your information will be kept confidential. However, if you share your email with us, it will be placed in a drawing for a gift card.

Hurry, the Community COVID-19 Impact Survey closes on Wednesday, June 15, 2022.

This survey is supported by a collaborative effort from the Social Development Commission, Milwaukee Consortium for Hmong Health Inc, Milwaukee Latino Health Coalition, United Community Center, and the Medical College of Wisconsin.
