What is the FIRST Program?
The Family and Individual Resources Support Team (FIRST) is a program designed to link you to other SDC programs and community resources.

How can I become a FIRST client?

  • If you are currently enrolled in a program through SDC: You can be referred through that program. Simply ask an SDC staff member with that program to refer you to FIRST.
  • If you are NOT currently enrolled in a program through SDC: You can call
    (414) 906-2814.

What are the services offered by the FIRST Program?
FIRST services may include, but are not limited to:

  • Linking you to SDC Programs and Community Resources.
  • Providing follow-up contact.
  • Completing an Assessment of Needs with you.
  • Helping you to identify your strengths and supports.
  • Advocating for you.
  • Providing Crisis Intervention and Support Services.

How can FIRST benefit me and my family?
FIRST can:

  • Link you to resources that will serve to improve the quality of life for you and your family.
  • Provide one-on-one specialized service.
  • Work with you to prioritize needs and develop long-term and short-term goals.
  • Provide follow-up contact to ensure that your need was adequately addressed.
  • Empower you to move toward selfsufficiency.
